Here's how:
Step 1. I already have this image ready for this tutorial, let's just use this for the mean time. Just copy this and open in Photoshop.
Photo credit is from this link.Step 2. Duplicate Layer or press Ctrl+J (Automatically, this will be our Layer 1.) Then, go to the Toolbox and grab the Rectangular Marquee Tool (M). In the Options bar, adjust the width and height to 7 x 5 inches.
Using the Rectangular Marquee Tool, we are going to make a selection. This will be the size of our photo where our image will pop out later.
Step 3. Next, duplicate selection (Ctrl+J). This will be our Layer 2.

Step 4. Click Create a new layer icon in the bottom of the Layers Palette. Now, go to the Tool box and select the Paint Bucket tool. Fill our new layer (Layer 3) with black. After that, drag this layer between Layer 1 and the Background Layer.

Step 5. Click Layer 1 because we are going to extract the figure in our photo which we are going to use as our pop up image. So, select Filter > Extract (Alt + Ctrl + X) or you can use the Lasso Tool or the Pen Tool to extract our image.
Note: We only need to extract the figure that we are going to use as our "pop out".

Refine the edges of the extracted image. Use the History Brush and the Erase Tool.
Step 6. Next, we have to increase the size of our canvas. Go to Image > Canvas Size... (Alt + Ctrl + C). Increase Width to 110%.

Step 7. Select Layer 3, right click thumbnail of Layer 2. 
Step 8. Select Edit > Free Transform (Ctrl+T). In the Options Bar, make the value of our Width to 110% and our Height to 113%.

Step 9. Grab the Paint Bucket Tool and fill layer with white.

Step 10. Select > Inverse (Shift+Ctrl+I) and press Delete key. Next, hit Ctrl+D to deselect.

Step 11. Select Layers 1, 2, 3 (Shift+Click the top layer). Press Ctrl+E to merge.
Step 12. Click Create a new layer. Grab the Paint Bucket Tool and with any color you like (I'm using black) fill it on our new layer. This will be our background color or if you have an image that you can use you can drop it here. Next, we have to drag our new layer between Background and Layer 2.

Step 13. This is now our finished product.

You can add a little perspective to our image.

You can also use another image as your background and add drop shadows to it.
